The person I knew who was into this shit was a "transman" for a while, and then somehow reverted back to being female again; I don't claim to get that, but that was part of being a "little", which is someone who roleplays being very young.
In her/his case, I get it. Their childhood was fucked up. Mom, Dad, and grandparents were all abusive; there was no one like "hey, I love you and I'm proud of you no matter what" in her/his life.
I saw the scars on his wrist one day and wanted to joke around like "Wow, did you really kill all those people?" but thought better of it.
