Binro the Heretic      I can still remember the grindhouse-style radio ads for this movie. They played a short audio clip of Belial (the deformed conjoined twin) killing one of his victims. The gravelly-voiced announcer said air sickness bags would be given to everyone in the audience.

Binro the Heretic - SPOILERS -
Dwayne, the guy you saw kissing the girl, was born with his severely malformed conjoined twin brother Belial attached to his side. When they were younger, their dad hired some less-than-reputable surgeons to cut Belial off Dwayne's side so Dwayne could live a normal life. Belial was supposed to have died in the operation, but survived, barely. Thanks to a telepathic link, Dwayne found his brother and kept him alive.
Years later, the brothers are hunting down the doctors who separated them.
If memory serves, this is the scene were Dwayne falls in love with a girl he met while searching for one of the doctors. Belial, being linked to Dwayne telepathically, knows what's going on. What you see here is Belial having a jealous tantrum. Being a deformed psychotic freak, he'll never have a girlfriend...or will he?
