This is the third movement of his opus 132 String Quartet. It is known as the "Heiliger Dankgesang" or "Song of holy thanksgiving"

The title ("Holy Song of Thanksgiving by a Convalescent to the Divinity, in the Lydian Mode") is Beethoven's own.
