I'd say neither, but I just lost 30 pounds by watching what I eat. Some of us don't have to, but some of us do.

I don't mean to sound unsympathetic. Obviously it sucks. But I find it irresponsible that a surgeon would skip asking the patient to at least *try* to lose some body fat. They might add some years to their life in the process. But I guess the customer is always right.

In America, the AMA has a long list of requirements that need to be met before this surgery should be performed. It includes a lot of more conservative treatments being tried for years before surgery is recommended. A reputable surgeon would follow the guidelines before suggesting it.

Also, it's very hard to lose weight in just the breasts alone. You can't spot reduce fat. For women who are already thin, trying to lose more weight can be dangerous. Some women with larger breasts are also big, so that option makes more sense.
